How caregivers use it

Most people print one, fill it in once, and tape it to the fridge, where any first responder is trained to look. Keep a second copy in a wallet or glove compartment.

Hand it to a babysitter, a home aide, or a visiting relative so they are never guessing in a crisis. Bring it to the ER so you do not have to recall every detail under stress.
