What's on the template

One page of patient details and a medication table, plus a continuation page with extra rows and notes. Designed for caregivers managing more than one or two prescriptions.

• Patient name, date of birth, last updated, emergency contact. Filled in once, the page works as a single source of truth for any visit.
• Allergies callout at the top. Visually distinct so an ER physician sees it first.
• Medication table: name, dose, when (schedule), purpose, prescribing doctor — the same five fields a doctor's office asks at every visit.
• 27 rows total across the two pages. Most patients fit comfortably; complex regimens still have room.

How caregivers use it

Most people print one, fill it in once, and keep a copy in a wallet or glove compartment. When a medication changes, they cross out the old line and add a new one in the next blank row, then reprint every few months when the page gets messy.

Bring it to every appointment. Hand it to the receptionist. The doctor's office no longer has to chase you for an updated list, and you no longer have to remember every dose under stress.
